Watch CEO VideoHello and welcome to the Community Care of Central Wisconsin (CCCW) website! My name is Jim Canales. I have had the pleasure and honor of serving as the inaugural Chief Executive Officer for CCCW since May 1, 2008.
CCCW became the Managed Care Organization (MCO) operating the State of Wisconsin Family Care Program in the counties of Marathon, Portage and Wood on July 1, 2008. Langlade County was added to our service region on 1/1/2011, and the county of Lincoln was added on 4/1/2011. Family Care provides a package of long term care benefits that support the identified outcomes of our participants. These participants, known as members, must meet functional and financial requirements of the Family Care Program and be either elderly, or be an adult with a physical or developmental disability, in order to participate.
CCCW has provided Family Care services to over 4,000 members since 2008. Over 700 different service providers are available to deliver services and goods to our membership.
I have worked in the field of long term care since 1977, and was employed by Portage County from 2000 - 2008 as their Director of one of the original five county pilot sites for Family Care, Community Care of Portage County.
I have been married to Nancy for the past 24 years, have two adult daughters, and own a 5 pound Yorkshire terrier and a 75 pound Goldendoodle. We reside in the Stevens Point area on a quarter acre lot that boasts a multitude of trees, shrubs and perennials that I tend to when not working. I also have the best volunteer job in the world….hosting/producing jazz, blues, and acoustic radio shows at the UWSP campus radio station, 90FM!
I am very pleased to say that CCCW continues to meet necessary State certification requirements while operating in the counties of Marathon, Portage, Wood, Lincoln, and Langlade. Nearly 3,100 members are enrolled and served on a daily basis by qualified care management teams and a certified contracted provider network. Member satisfaction surveys have been conducted in each of the last three years - the survey results consistently show a high level of member satisfaction with the services members receive from their care management teams and their direct care providers.
CCCW serves members throughout the region from our offices located in Stevens Point, Wausau, Wisconsin Rapids, Marshfield, Antigo, and Merrill. An organizational Mission Statement and set of Organizational Values that define our core beliefs and serve to support the strengths and preferences of our members have been developed by our Board and staff, and are available for viewing elsewhere on this website.
