Community Care of Central Wisconsin (CCCW) is a state-certified Managed Care Organization. We are contracted through the State Of Wisconsin Family Care Program to support the health, medical and long-term care outcomes of our members who are elderly or adults with physical and/or developmental disabilities. Our service region includes the north central counties of Langlade, Lincoln, Marathon, Portage, and Wood, and the counties of Ashland, Barron, Bayfield, Burnett, Douglas, Iron, Polk, Price, Rusk, Sawyer and Washburn in northwest Wisconsin. Fourteen (14) CCCW offices are located throughout the service region, with the Administrative Center being located in Stevens Point. Over 5,400 members are currently served by CCCW on a daily basis.
CCCW has developed and maintains a state-certified network of providers to support the identified outcomes of our members. “Providers” is a general term used for organizations and businesses that provide health, medical and long-term care supports and services to our membership. Over 1,400 providers currently are under contract with our organization. CCCW is committed to ensuring that our provider network has the adequacy and capacity to support the outcomes of our members, and maintain a constant interest in expanding our network size and scope.

1-2 Bed AFH Webcast Training and Exception Reports
The Department of Health Services (DHS) created a website dedicated to the Wisconsin Medicaid Standards for Certified 1-2 Bed Adult Family Homes. The website can be found at www.dhs.wisconsin.gov/regulations/afh/1-2bed. Additional items have been posted on the website as follows:
1. The webcast training for the 1-2 Bed Adult Family Home Standards; and
2. Report Exceptions to 1-2 Bed Adult Family Home Standards (for certifying agency).
