Community Care of Central Wisconsin (CCCW) is the Managed Care Organization contracted and certified by the State of Wisconsin to provide eligible residents of Marathon, Portage, Wood, Lincoln, and Langlade counties the benefits, services and goods associated with the Family Care program.
Community Care of Central Wisconsin coordinates quality, costeffective and individualized long-term care supports available through Wisconsin's Family Care program that support member strengths, needs, and the development of meaningful community connections that lead to full citizenship in their communities.
